Plan

Records review

Send your relevant records ahead of a virtual consult so the clinical team can assess candidacy safely.

What to gather

  • List of current medications (name, dose, frequency).
  • Any relevant urology or sexual medicine records.
  • Prior surgical reports (prostate, pelvic, vascular, penile).
  • Recent labs if available: CBC, metabolic panel, HbA1c, total & free testosterone, PSA.
  • Cardiology clearance letter if you have known coronary artery disease.
  • Photos are not required at this stage.

How to send

Records go directly to the clinical coordination team via a secure channel provided after you submit the consultation request. This site does not store PHI in browser storage or in any public form.

What happens next

  1. Records reviewed by the clinical coordination team.
  2. A virtual consult is scheduled (typically 20–30 minutes).
  3. Candidacy, options, cost, and travel timeline are discussed.
